We’re excited to hear that you are considering Mayfair Christian School!  You will find everything that you need to get started with the application process on this page.  Also, you can always browse our library of documents if you know what you’re looking for.

To help you along in the process, an enrollment checklist document organizing of all the necessary forms is available for you to use.

If you have questions about any of the steps, please feel free to call the school at 330-896-3184.  Over the summer, please leave a message.  Messages will be checked periodically and someone will get back to you as soon as possible.
Step 1
Fill out and submit an Application for Admission.  If you are registering a returning student, fill out a Re-registration Form.

Step 2
If you need financial assistance, please fill out a Financial Aid Application.  Financial aid is awarded based on need and is given on a first come, first served basis.  The application MUST be filled out completely, including the amount per month that you are able to pay and the amount of tuition assistance you are requesting.  (These two figures should add up to the total tuition amount.  Please see the financial information for the current tuition rates.)  You must also attach your income tax return (1040) from the previous year or other proof of income.

Step 3
If your child qualifies for the Ohio Ed Choice Scholarship, fill out the application and submit it to the school.  Information on how to qualify, as well as the application materials, can be found on the Ohio Department of Education website.

Step 4
Provide copies of the child’s birth certificate, Social Security card, proof of medical insurance, and immunization records to the school.

Step 5
Fill out a Request for Transfer of Records and return it to MCS.  We will send the form to your child’s former school.  Students starting Pre K or Kindergarten do not have to complete this step.

Step 6
All new students, and students entering Kindergarten, 1st, 4th grade, and 7th grade must have a physical exam.  Please schedule an appointment with your child’s doctor and have them fill out the medical exam form.  Some doctors have their own form.  This is acceptable as long as it includes the same information.  If you have not given the school a copy of your child’s immunization record, be sure to get a copy from the doctor.  Returning students should give the school an updated immunization record whenever any immunizations are added.

Step 7
Please complete, sign, and return to the school copies of the Parental Pledge, Student Pledge, Internet Access Agreement, Handbook Confirmation Receipt, and the Consent To Treatment Form.

Step 8
Supply lists for each classroom will be provided over the summer.  There will be an open house/back to school night just before school starts.  Please plan to attend to learn more about the school policies, classroom expectations, and receive an updated copy of the handbook.

Step 9
A financial plan must be approved and signed by the parents and the school treasurer before a child can begin school.
